Safety Zone Entrance Criteria

Judging

  • UAC’s Safety Committee will evaluate each entry. The judges may choose to not make an award in any category, based on the merits of the entries. All judges’ decisions are final.

Eligibility

  • Only current UAC business-level members may submit entries.
  • Members are encouraged to submit entries for any or all of the award categories.

Entrance procedures

  • Application materials must be completed and submitted properly according to entrance criteria and submission instructions (below) to be considered for a UAC Safety Zone Award.
  • Improper submission may result in disqualification.
  • There is no entry fee but you must be a current UAC business-level member.
  • Awards recognize safety performance from January 1 – December 31.

Awards

Prizes will be as listed below for each award category. In addition, winning companies will have a Safety Zone Winner badge placed on their UAC website profile and also can include that badge on their company website. Safety Zone winners will be featured in an issue of UAC Magazine.


Safety First Awards

The entry with the lowest number of incidents will be awarded a certificate (one winner in each size category). If one company wins all three certificates, that company will receive a Safety First plaque.

Vehicle Safety:

  • Goal: to have the lowest number of work-related at-fault motor vehicle accidents for the number of vehicles you have on the road, when compared to other entries in your company size category. Complete the entry form and submit with your Vehicle Loss Run Report Summary.

Health:

  • Goal: to have the lowest number of work-related OSHA recordable injuries or illnesses for your company size, when compared to other entries in your company size category. Complete the entry form and submit with your OSHA form 300A.

Lost Time:

  • Goal: to have the lowest number of work-related injuries or illnesses that result in any lost time by workers, work restrictions or job transfers for your company size, when compared to other entries in your company size category. Complete the entry form and submit with your OSHA form 300A.

Safety in Action Video Awards

The top three videos will be recognized with Gold, Silver, and Bronze awards and will be featured on UAC’s Safety Zone website. Winning companies will receive a trophy.

Entrance criteria and submission instructions

  • Camera: This does not need to be a professionally produced video. Hand-held cameras, iPhones, and iPads are acceptable to use.
  • Videos: You may enter a maximum of three videos.
  • Length: The video should be between 30 seconds and two minutes in length.
  • Releases: You must get a signed release (see sample below) from everyone appearing in the video and provide those releases to UAC if your video is chosen for the website. Not having releases will disqualify your entry. You may download and use this release template.
  • Format: Save the file as  a AVI, FLV, WMV, MOV, or MP4 (MPEG4).
  • Pointers: Your video should be informative and engaging. Involve your staff; show employee/employer interaction. Make key points by conveying practical information and use hands-on examples. Demonstrate safety best management practices.
  • Submission:  Complete the entry form and submit your video(s) or provide the URL if the video is already online at youtube.com. Maximum file size for upload is 50 MB; if your video is larger, please post the video at youtube. com and supply us with the URL.

Culture of Safety Photo Awards

The top three entries will be recognized with Gold, Silver, and Bronze awards and will be featured on UAC’s Safety Zone website. Winning companies will receive a trophy.

Entrance criteria and submission instructions

  • Photos: Submit a maximum of eight photographs. Your entry will be judged on the group of photos submitted, not on individual photos.
  • File format: Photos can be in either .jpg or .png file format.
  • Releases: You must get a signed release from everyone appearing in the photos and provide those releases to UAC if your entry is chosen for the website. Not having releases will disqualify your entry. You may download and use this release template.
  • Pointers: Photos can include any or all of the following: safety features, layout, organization, finishes, ventilation, chemical storage, PPE use, signage, equipment, machine guarding, lock out/tag out, and/or vehicles – or anything else that your company does to promote safety.
  • Submission:  Complete the entry form and submit your photos.
